Planificación y Referencia de Dar de Alta

Planificación del Alta

Los pacientes se evalúan en el momento de la admisión para posible planificación de medios de alta y asistencia.

Las referencias de planificación del alta pueden iniciarse a través de una orden sometida en el formulario de Ordenes Médicas. Al recibir la referencia, el administrador de casos realizara una evaluación y comunicara el plan en las notas de progreso.

Comunicación del Médico Referente

Al momento del alta, el médico referente deberá recibir un Resumen Post-visita vía fax que contenga la siguiente información:
  • Diagnóstico
  • Procedimientos realizados
  • Instrucciones del alta, tal como medicamentos y citas de seguimiento
  • Nombre de la agencia de salud del hogar
  • Nombre de una compañía de equipo médico de confianza
